053592 11/05/04, 1:33 PM PST Provisional ballot problem; Other ballot-related problem; Other Fellowship of Believers Church; 3333 12th St;; Sarasota, FL, Sarasota, Sarasota County, Florida Caller witnessed old white female (approx 90 yrs old) who was called by a person alleged to Supervisor of Board of Elections in Sarasota to send in her voter registration card because they needed to issue another one. Shen she came to the polls, officials told her that she had already voted. They issued her a provisional ballot.

018589 10/31/04, 12:18 PM PST Machine problem Pasco County Court House, Dade City, Pasco County County, Florida The electronic voting machine did not function correctly. The voter placed her vote for president and when she confirmed the vote for president a different name came up (Harris). The poll assistants were able to correct the mistake and the voter was able to vote for the correct candidate. Her final vote was for the correct candidate.
