A neighbor called the phone numbers given on the flyer, and the person she spoke to just took the information and said they'd call back, but they didn't call back.

She was asked to fill out a yellow envelope (w/ precinct d on the outside) with her name and address, then she voted in a machine and put her completed voting card into the yellow envelope and they put it in the regular box. when she got home, she found her reg. card which said precinct k. she went bac, showed them her card at (7:15 pm after polls closed, they said "oops" one worker muttered "everybody was using that list" Called board of elections to see if her card was right, list was wrong. the democratic monitor had her fill out a form to document the mistake.

Voter is a New York resident who previously lived in Ohio (Hamilton County), has an Ohio driver's license, and whose parents live in Ohio. Voter had previously registered, in prior elections, in Ohio. Voter sent an application for an absentee ballot to Hamilton County in late August / early September. Voter called Hamilton County Board of Elections in late September to ensure she was still registered, and official confirmed that she was. Voter then received a letter on October 5, which was dated October 4, stating that she was not registered and that she must register to obtain an absentee ballot. The October 4 letter stated that the deadline for registering was October 4. Note that the county had received her application at least a month earlier. Voter called the county back on October 5 to figure out what happened, but the phone number (which had been listed on the October 4 letter) did not work or she was hung up on. She then sent an e-mail, also on October 5 or 6, to the author of the October 4 letter (whose name is perhaps Deborah), but she has yet to receive a response. Voter reported that her sister had similar problems with an absentee ballot. (Voter recognized that she may not be entitled to vote in Ohio given her New York residency; but she wanted to report the incident as a problem with Hamilton County.)

Issue 1: David submitted a change of address and registration about 4 weeks ago. He went to the polling place indicated on his registration card, where he was told that he was at the wrong district. They didn't have his name on the list, and did not initially ask him for id or his registration card. He was sent to another line, where they did ask to see his identification and registration card. They confirmed that he was, in fact, in the correct voting precinct, and had him fill out two forms to confirm his address (possibly an affadavit? my interpretation)These forms, along with his ballot were placed in a different colored envelope. I think this may have been a provisional ballot but they NEVER told him that it was -- though he does know it wasn't a standard ballot. Issue 2: This gentleman also had an issue with actual voting. He said that on page 2 of his ballot, all of the candidates were unopposed. This was not the case with page 3. 3400 provisional ballots were rejected,  the reasons were not fully determined.    But  "Approximately 400 of those [Hamilton County provisional ballots], he thought, were rejected because they were cast in the wrong precinct, despite the fact that they were cast at the right polling station (i.e., at a polling station with more than one precinct)."  (ie, there was malfeasance on the part of poll officials and the votes were from legal voters and should obviously have been counted)
